Certificate Order Form
We
request information from the customer on our order form. Here
a customer must provide contact information (such as "name", "telephone
number", "email address", and "billing address") and financial
information (such as "credit card number", and "expiration date").
This information is used for billing purposes and to fill customer's
orders. If we have trouble processing an order, this contact information
is used to get in touch with the customer. The information is
not encrypted, however after any information is received by Rae
West Massage Therapies, Inc. it will be treated as described in

clients are given the opportunity to 'opt-out' of having contact
